請假申請表 Leave Application Form

同學在缺席課堂或活動時，必須提交請假申請表並附上證明(如醫生紙)。

Students are required to submit the leave application form to HKU SPACE ApL Aviation Programme Team for lessons/activity that are absent. Supporting documents are required for proof (e.g. Medical Certificate).
